It's really easy to get the action into a condition where the axis lines the mounts sit on aren't paralell.

Very true.
Even if the manufacturer produced two perfect scope rings, they still have to be attached to the rifle action and misalignment to some degree is pretty well a sure thing.
You can often get away with very small misalignment on a scope that stays put, but on a QD scope system I believe lapping is very important.
